Ten journalists at The Record of Bergen County, NJ, spent eight months investigating the actions of Ford, government officials and even the Mob in exposing residents of the woodlands of northern New Jersey to millions of gallons of toxic paint sludge carrying lead, arsenic and other dangerous containments.
___ The Entry Form filled out completely, with appropriate signature(s).
___ A cover letter or concise summary (approximately 250 words) describing the entry submitted for Prize consideration. If appropriate, the summary must describe all web-based components (e.g., video, text, audio, etc.) to clarify the scope of the entry for the jury.
___ Short biography of each entrant listed on the Entry Form.
___ Audience challenges to the substantive content of the submission, if any. These might include subsequent letters to the editor or published or broadcast corrections.
___ The journalism entry (see acceptable formats for details). Entrant is responsible for assuring electronic submissions are readable in both PC and Mac formats. Entrant must provide access to online stories or series, including a username and password if required.
___ One copy of completed Permission Form for each entrant listed on the Entry Form. The Permission Form should be downloaded from the Grantham Prize Web site, printed, signed and included in the package. Only one copy of this form is required.

___ Check or money order in the amount of US$40, payable to Metcalf Institute.
___Sender should request that the postmark be printed and affixed to the package.
